The Role of the Clinical Competency Committee in EPA-Based Learning

In Competency-Based Medical Education (CBME), moving away from traditional time-based training toward entrustment-based progression changes everything about how we assess trainees.

While daily observations give us great snapshots of individual performance, granting a trainee real autonomy requires a much broader view. That is where the Clinical Competency Committee (CCC) comes in.

Let’s dive into what the CCC actually does in daily practice, why it matters, and how its bird’s-eye view differs from daily workplace assessments.

Connecting daily snapshots to the bigger picture

At the heart of EPA-based learning lies a simple distinction: the difference between daily clinical observation and formal entrustment.

During clinical rotations, Workplace-Based Assessments (WBAs), like mini-CEXs or procedural skill observations, are all about real-time feedback moments. A trainer watches a trainee perform a specific activity, gives instant feedback, and logs a snapshot of that moment. These assessments are purely formative: they are meant to help the trainee improve on the spot.

However, relying solely on individual snapshots to grant autonomous practice is risky:

WBAs vs. the CCC: A Complementary Dynamic

To understand how the clinical learning environment works, it helps to view WBAs and the CCC as two sides of the same coin. They don’t replace each other; instead, they serve completely different but complementary purposes.

WBAs are done in real time by individual clinical supervisors on the floor. Their main goal is formative: giving immediate, practical feedback on specific tasks and activities so the trainee can adapt and learn on the spot.

The CCC, on the other hand, meets periodically as a panel of educators to look at the bigger picture. Their goal is summative: reviewing all accumulated evidence over time to grant formal entrustment levels, spot trainees who need extra support, and ensure consistent standards across the entire cohort.

This division of roles does something crucial: it protects the learning culture on the floor. When clinical trainers know their daily notes contribute to a broader dataset, rather than acting as a make-or-break decision, they feel much freer to give honest, constructive feedback.

Trainees benefit too. They feel safer asking for help and showing vulnerability on the job, knowing that their formal entrustment rests on their overall progress over time, not a single off-day.
